Guidelines Of Facility
The Evergreens
For the mutual benefit and safety of all residents of The Evergreens, the following guidelines are in effect:
- Meals
Breakfast: 8:00-10:00 am
Lunch: 11:00am-1:00 pm
Dinner: 4:00 pm-6:00 pm
Residents may choose to have meals outside of the times listed as noted on the resident’s Individualized Service Plan.
Residents may invite guests to The Evergreens for meals with one day advance notice. Prices as follows:
Noon: $10.00
Evening: $10.00
Holiday Dinner: $10.50
- Personal laundry, bed linens and towels are cleaned on a weekly basis or as needed. Residents are expected to wear a clean change of clothing and change as necessary.
- It is suggested that residents bathe at least weekly.
- To ensure safety of all residents in the building, individual resident suites must be free of clutter to avoid safety concerns.
- Smoking is prohibited in ALL areas of The Evergreens, including e-cigarettes.
- Residents are required to sign in and out when leaving and returning to The Evergreens.
- All electrical fixtures must be approved by the Assisted Living Director/designee, to ensure resident safety and fire protection for all residents.
- Individual resident room keys are available.
- Residents are asked to bring with them, upon admission to The Evergreens: *Personal toiletries: e.g. toothbrush, toothpaste, all personal items, disposable undergarments.
- Residents are encouraged to bring personal items for their suites, such as pictures and other items
- Residents may not bring or permit others to bring any prescription medicine, over-the-counter drugs, or alcohol into The Evergreens without a written physician order.
- Residents must be appropriately dressed while in the common areas of The Evergreens. This includes ensuring that personal hygiene habits have been observed.
- All food items in resident suites need to be in lid covered containers.
- A resident or their legal representative (Guardian or activated Health Care Agent) may withdraw an individual’s visiting privileges at any time.
- Residents may have visitors as permitted or deny.
- Visitor Responsibilities:
- Visitors should not visit if they are feeling ill.
- Visitors must follow transmission-based precautions (wash hands, cough into sleeve, etc.) and use personal protective equipment (gloves, masks, gowns, etc.) as instructed.
- Visitors may not interfere with the plan of care or the provision of care.
- Visitors may not compromise DQA requirements.
- Visitors must be considerate and respectful of residents, other residents and their visitors, staff, facility, and other resident property.
- Visitors must cooperate with disaster plans and drills.
- Visitors could be asked to leave if:
- Resident requests.
- Visits are medically contraindicated (the resident is contagious and/or the visitor is contagious.
- They are inebriated or disruptive.
- They endanger the safety of the resident, other residents, or staff.
- They are disturbing or abusive to the resident, other residents, or staff.
- There are any other issues that could be deemed dangerous or have a negative impact on the resident.
