1. The Evergreens contracts with an outside provider for Beautician/Barber Services.  The Salon is located with Hillview, and easily accessible to residents of The Evergreens.  Listed below are standard service offered by the Salon provider:

    

   • Shampoo only:  $10.00
   • Haircut (basic):  $17.00
   • Shampoo & Set:  $18.00
   • Permanent (includes cut & set):  $60
   • Hair Color (includes set):  $50
   • Beard/Facial Trim:  $7.00
2. The Evergreens welcomes residents to have guests for meals.  We ask for a 24-hour notice so ensure proper quantity of food is prepared.  Guests may pay for meals at the front desk of Hillview.  The following are the charges for guest meals:
   • Lunch:  $10.00
   • Dinner:  $10.00
   • Holiday:  $13.00
3. Certain optional or extraordinary services are not included in the monthly rate.  Service may include:

• Additional RCA Assist:  $25 per 15-minute increment
• Additional laundry services (beyond weekly standard):  $130/month
• Additional RN Services:  $150/hour
• Assistance with moving suites:  $50/hour
• Lost key fee/REKEY:  $50.00
• Additional Key:  $5.00
• Lost, broken, misplaced pendant:  $100
• Tray Delivery Service:  $7.50/time

Effective November 1, 2025

Daily All-Inclusive Rate:  $225 day

Residents will be charged a one-time non-refundable application fee of $750.

Rate includes the following services:

• Supportive care services such as housekeeping and laundry
• Three meals each day plus snacks
• Opportunities for leisure per the resident’s Individual Service Plan
• Qualified staff in the facility 24 hours a day
• Medication management and administration assistance if needed
• Assistance with showers or whirlpool spa baths as needed
• Modernized call system for easy access to caregiver staff 24 hours a day
• Assistance with activities of daily living (ADL) including dressing, grooming, and toileting as needed
• Daily recreational programming as determined by client interests
• Antenna provided with Smart TV and Wi-Fi
• Room and board

The rate does not include:

1. Medications
2. Physician, Psychologist, Psychiatrist Visits
3. Oxygen Therapy, Pulmonary Treatments
4. Transportation Services
5. Newspapers
6. Telephone
7. Cable Services
8. Salon Services
9. Personal Supply Items
   (i.e., toothpaste, toothbrush, denture care items, incontinence products, etc.)
10. Personal clothing
11. Dry cleaning
12. Disposable medical supplies
    (i.e., catheter and ostomy care items)
13. Speech Therapy
14. Occupational Therapy
15. Physical Therapy
16. Durable Medical Equipment
17. Costs associated with community recreational activities, e.g. event fees, movie tickets, other recreational activities of the tenant’s individual choosing not associated with the facility’s sponsored event
18. Dietary costs associated with allergies and personal preferences
